Stir together water, apples, oat groats, apple juice, lemon zest, cinnamon and nutmeg in slow cooker or crock pot until thoroughly combined.
Set slow cooker to low, and cook 4 to 6 hours, stirring occasionally. Add water to thin, or cook longer to thicken.
Serve hot, topped with brown sugar and raisins, if desired.
